For your convenience, you can now print out and complete your forms at home before visiting North Georgia Surgery and Comprehensive Breast Center. Although it’s not required, bringing completed forms will save time and help make your appointment fast and easy.
You can bring your completed forms with you or fax them to (706) 517-3983. If you have any problems with the forms, simply call our office and we will gladly email or fax them to you.

In addition to the new site being easier to read and navigate, it is also much easier for us to update.
